Requirements are usually documented in the BRD, software requirement and specifications (SRS) document, or in the Requirements section of the project management tools, such as Jira, being used by the team. Test scenarios and test cases are created in order to verify the stated requirements. It's important to ensure and track that all the requirements have corresponding test cases and are covered in the testing and verification phases.
