The SNFVBP is scheduled to start in 2019. It will base Medicare reimbursement from the government to SNFs partially on two outcome-related measures:
- The 30-day all-cause readmission rate
- The 30-day potentially preventable readmission rate
The rates apply to residents of SNFs who are admitted to other hospitals. When this program begins SNFs could benefit from collaborating with machine learning practitioners who predict which patients are at risk for readmission.
