Model-driven apps are created by assembling components from the Common Data Service. You customize the individual components and then select the appropriate components to meet your business requirements.
Model-driven apps are created from the Power App maker portal (https://make.powerapps.com). To create an app, click on the + Create button in the maker portal and click on Model-driven app from blank, and then click on Create. The following page will appear:

In the Create a New App page, enter the name of your app. Unique Name is automatically populated. Click on Done.
If you are using solutions, you can check the Use existing solutions to create the App option. You will then be able to select a solution and the components in the solution will be added to the app.
After clicking Done, the App Designer page will open, as shown in the following screenshot:
